Long Term Care Placements, Elder Care Home Placements, Assisted Living, Respite Care and Independent
I will discuss placement options with you. I will tour locations with you upon request and help to ask appropriate questions to fit the needs of yourself or your loved one. These are not decisions individuals make everyday. I will guide you through this.
In-Home evaluations
My services are completely "needs based". I will visit your home to collect information to find the best options that will meet your needs. If you or your loved one are in the hospital setting and need placement or services I will accommodate.
Private Duty, Home Health, Palliative Care, and Hospice Care recommendations
I will educate you on the differences, and benefits of all levels of care. Remember, it is about finding the right level of care at the right time to maximize support. Most entities are rated with star ratings and quality scores. We will look into these together. There are a lot of options to utilize that are covered by insurance benefits.
What are the fees for service?
I do not charge my clients for services. I contract with various communities that reimburse me for my services.
How long do you receive services?
I will be available to you until your families needs are met. I will keep your profile for up to 12 months after placement. You can always initiated services when needed.
